What is adult medical day care?
It is a licensed daytime program for adults who live at home but need health oversight and supervision: nursing assessments, medication monitoring, therapy, meals, activities, social work and usually transportation. Maryland's Medical Day Care Services Waiver describes it as at least four hours a day, up to seven days a week.
For a family caregiver who works or simply needs the day, it can delay or replace a move to assisted living.

Many centers run vans within a defined service area, so the center's address matters less than whether it picks up on your street.
Who pays for adult medical day care?
Maryland Medicaid pays through the Medical Day Care Services Waiver for people who already have full Medicaid, need a nursing facility level of care and have a medical order for medical day care; the Community Options Waiver also lists medical day care among its services. Otherwise families pay privately -- the 2025 CareScout Maryland median is $117 a day, about $2,526 a month for weekday attendance. Some long-term care insurance policies cover adult day services.
How do you choose a center?
- Visit during the program day, not at closing time.
- Ask about the nurse-to-participant ratio and how medications are handled on site.
- Ask about the pickup window and ride length.
- Ask how the center handles a participant with dementia who tries to leave.
- Check the license number on OHCQ's adult medical day care directory.
Who is a good candidate for adult medical day care?
Typical participants live with a spouse or adult child who works or needs a break, need help with medications or have health conditions that benefit from regular nursing checks, and are lonely or under-stimulated at home. People with mild to moderate dementia often do well with the structure, provided the center is prepared for wandering risk.
Adult medical day care is less suitable for someone who needs care overnight, or for a person who becomes very distressed by transitions. A trial week often answers the question better than any assessment.
Ask the center for a sample daily schedule. A good one shows a balance of health services, meals, exercise and activities matched to the group's abilities.
How does it combine with other services?
Many Baltimore County families combine weekday day care with a few hours of in-home help in the morning and evening. Others use day care for a year or two and then move a parent into assisted living when nights become unsafe. Day care staff often notice changes early and can be valuable partners in that decision.
If the participant has full Medicaid, Community First Choice may pay for personal assistance at home on the days or hours the center does not cover. Maryland Access Point can explain how the programs fit together.
Hospice patients can sometimes continue attending day care; ask both the center and the hospice how they coordinate.
What should you look for on a visit?
Arrive mid-morning and watch for twenty minutes. Are participants engaged in something, or sitting in front of a television? Is the nurse visible? Are staff speaking to participants respectfully and by name? Is the space clean, with enough room for walkers and wheelchairs? Is lunch something your parent would actually eat?
Ask how the center communicates with families -- a daily note, a weekly call, a portal -- and how it handles a participant who falls ill during the day. Ask how many participants attend on a typical day compared with the licensed capacity, and what the staff-to-participant ratio is.
Finally, ask about the first week. Good centers have a routine for helping new participants settle in, including shorter first days and a familiar staff member assigned to greet them.
How does Baltimore County's supply compare with the rest of the metro?
Baltimore County holds 43 of the metro's 81 licensed centers, Baltimore City 21, Howard County 8 and Anne Arundel 7, with very few in Harford and Carroll. Families on the edges of the metro sometimes cross county lines for a center whose van covers their street.
Where can you find every center?
Our adult medical day care directory lists every licensed center in the metro with capacity and license number, and the Baltimore County Department of Aging (410-887-2594) can explain Medicaid eligibility and other respite options.
